The NHS Health Check programme aims to help prevent heart disease, stroke, diabetes and kidney disease. Everyone between the ages of 40 and 74, who has not already been diagnosed with one of the conditions below, is invited to have a check to assess their risk of heart disease, stroke, kidney disease and diabetes and will be given support and advice to help them reduce or manage that risk.

  • CHD
  • Chronic Kidney Disease Stage 3 or greater
  • Stroke
  • Hypertension
  • Transient Ischemic Attack
  • Diabetes
  • Arterial Fibrillation
  • Peripheral Vascular Disease
  • Heart Failure
  • On Lipid Lowering Therapy and issued a prescription in the last two months
  • Previously diagnosed CVD risk >20% & being reviewed annually
  • Patients who have had an NHS Health Check since 1st April 2009

If you already have any of the above conditions you should be under the care of your GP on a regularly basis and therefore are not eligible for a free NHS Health Check.

What happens at the check?

This check is to assess your risk of developing heart disease, type 2 diabetes, kidney disease and stroke.

The check will take about 20–30 minutes.

  • You’ll be asked some simple questions.
  • For example, about your family history and any medication you are currently taking.
  • We’ll record your height, weight, age, sex and ethnicity.
  • We’ll take your blood pressure.
  • We’ll do a simple blood test to check your cholesterol level.
